The Endoscopia book clue
During the late hidden-route sequence, Mellow reaches a small office-like room with a book. PurpulPancakes' achievement playthrough records the book as Endoscopia and Mellow reading the word as Endacopia at 36:49–36:52. Big Leaflet's story recap discusses the same reveal at 25:30.
That scene gives you a direct narrative link between the title and the book. You do not need to invent separate definitions for the title's syllables to make the reference work. Automatic subtitles can distort character names and spellings, so the displayed book remains the useful visual check.

Why looking inside fits the game
The house, toilet passages and later mouth scene turn familiar rooms into parts of a larger physical setting. Ending A makes the escape through that body's mouth visible. Ending C returns to the Surgeon's work and the book, then moves toward the ritual.
The title therefore has a concrete story connection: you have spent the adventure moving through an inner world. Reading it as a journey into memories, fear or Mellow's search for a home is a further interpretation. Those readings can fit particular scenes without becoming another officially defined meaning of the word.
